INSTRUCTIONS FOR MEDICAL DECLARATION ON PHONE AT SCS BUILDING

You can quickly declare in 4 steps as follows:

Step 1: Open your phone's camera or using Zalo app and scan the QR code.

Step 2: Scanning QR code. Click on the notification displayed at the top of the screen to open the QR code's web

Step 3: After accessing, filling out medical form step by step

Step 4: Finally, click "Submit" at the bottom of the page to complete the medical declaration.

Thank you for making the declaration according to the regulations at the building.
